The following is an outline of an online marketing campaign I would undertake for your book.
Firstly, you should create a page for your book on your site similar to the one at http://www.livingplaces.com/~atccp.html
Also, I would follow the lead of excerpting a small section or parts of sections to give a flavor for the book.
and
Add a guest book link to allow people visiting the book page a chance to leave their comments.
After the page is up I would do the following.
1. Create an electronic press release
2. Use an internet press release service to circulate it to proper outlets hoping it will be picked up.
3.Periodic Posts to Usenet Newsgroups:(once a week for a few weeks depending on how active the groups is)

4. Joining and posting to appropriate email discussion groups
Gerry, there are many groups who would be interested in knowing about your book. Go to the site http://www.reference.com/ and search for the the term "pain" in the mailing list directory to see 6 pages of forums devoted to discussing this topic. This does not take into consideration searches for groups with the keyword "nutrition" or "vitamins", "foods" or "health".
To get more of an idea of other email discussion groups, search for the same terms and notice the results from the site which allows you to search the NEW LIST archives at http://LISTSERV.NODAK.EDU/scripts/wa.exe?S1=new-list
This a time consuming process as you have to join each group, monitor it for a time to see if our post would be appropriate and if so, subscribing and posting it an then un subscribing from the group.
However, the above strategy selects a target audience to inform about your book.
5. URL Registration Campaign
Create meta tags for the page allowing for good search engine placement and registering it with 30 3 star engines as well as using a submission robot for submission to an additional 180 directories.
I don't know what you had in mind for a budget for marketing. I suggest we give it a try for 20 hours and see what results are produced. The cost would be $1000.00
